Already have a Yellow Card, or a Positive Notice Exemption Card? Great, please upload it to your profile.
Don’t have one?
There are two pathways for getting a Yellow Card:
-
If you ARE NDIS Registered as a Sole Trader - you can use one of these two methods:
- Apply for a Yellow Card using this form here then follow these instructions issued by the Department of Communities to lodge your application.
- If you already have a current Blue Card that's valid for 3 months or more then you need to apply for the (free) Yellow Card Exemption Notice, which you can access here.
-
If you are NOT NDIS Registered as a Sole Trader, and you do have a valid Blue Card, then you need to apply for a Yellow Card exemption via an eligible service provider that you are engaged by, either as a contractor, volunteer, or paid employee.
- You can find this form here.
PLEASE NOTE: For Care Online cannot act as the eligible service provider in this case, as we do not employ or contract you).
